This is my family's account of events that occurred just before and after the death of our mother. My mother's name is Ursula Norine Eloise Brathwaite. Her residence was in Palm Bay, Florida. I was her primary caregiver before she became ill. I brought my mother to New York when my wife was about to deliver our last child. At that time, she was intellectually intact.
During her time in New York, she became ill and was diagnosed with failure to thrive and severe dementia. She was subsequently placed in a nursing home. My cousin Charles Wilson was an employee of this nursing facility. The Facilities name is Menorah Nursing and Rehabilitation Center located in Brooklyn, New York.
A good and longtime friend of my mother, Ms. Olga, who also resided in Palm Bay, Florida visited my mother at the facility.
I believe she did not have my siblings or my contact information. Ms. Olga is approximately the same age as my mother. They are of the same generation. Both are in their eighties. Both grew up in the same community, of origin are Guyana, South America.
Charles Wilson, as I mentioned above, convinced my mother's friend (Ms. Olga) to have her daughters (Gail Adams and Pam Jackman Brown) give him Power of Attorney.
The Power of Attorney was signed by several individuals. Most importantly are three of the signatures on the document. One is that of the notary, the second of the witness and the third is my mother's (Ursula Brathwaite).
There was a second set of documents. This set of documents is of the sale of 1297 Asheboro circle SE, Palm Bay Florida 32909. The seller's affidavit, Dated April 13, 2018. This was notarized by Gail Adams, Honorable Judge of New York.
The issue at hand is my mother was cognitively impaired. On the admission to the Nursing home on 01/05/2016 her diagnoses included failure to thrive, severe Alzheimer’s disease, vascular dementia, major depressive disorder, mood disorder, dysphagia, gastrostomy and sacral ulcer.
This was a medical diagnosis, providing a picture of her physical condition. Ursula Brathwaite couldn't eat nor feed herself. The evidence of the sacral ulcer indicates she could not turn or reposition herself. These diagnoses were upon admission to the nursing facility on 01/05/2016.
There are clearly signatures on these documents that falsely and fraudulently represent my mother's signature.
Charles Wilson at that time began to financially abuse my mother. He began writing checks to himself and to his significant other. He sold my parents' home in Palm Bay, Florida utilizing these fraudulent documents.
This occurred with the signature of two New York state employees. One elected, one unelected. Both signatures were on the Power of attorney and one the bill of sale.
A police report was made at this time, with the allegations of financial abuse. The representative of the N.Y Police Dept. Detective Greene (66th Pct.). This Detective began an investigation into the accusations. After months of correspondence back and forth with the detective, he informed me there was nothing he could do. He stated this is a civil matter. This is despite providing him with copies of my mother's financial disclosures.
Justice was and will not be served. Due to the involvement of these two public officials.
I have copies of all documents (Power of Attorney, Bill of sale Florida house, my mother's medical records, bank statements and fraudulent checks, police complaint # 2019-066-000739).
The Power of Attorney was revoked by a Judge in Surrogate Court in Brooklyn, N.Y. Honorable Judge Lawerence Knipel on November 7th, 2018. This Judge was the overseer of the proceeding because every assigned judge to this proceeding recused themselves. No judicial officer wished to assume responsibility for handling this hearing. A clear conflict of interest. The purpose of the hearing scheduled for this date was to establish competency for Ursula Brathwaite. The result was guardianship. Guardianship was given to Tonya Hobson Williams and the attorney from Alter and Balbaro that was paid for with a fraudulent check from Charles Wilson (dated 05/21/2018).
Two signatures on the Power of Attorney and the bill of sale for my mother's property belong to New York State officials. One elected and one is an appointed official. Both signed documents claiming to witness a signing of a document that was impossible to witness due to my mother's contractures and cognitive limitations. These two signatures belong to the daughters of my mother's friend, Ms. Olga. Both are Judges and are responsible for holding other individuals responsible for violations of the law. The exclusive duty of these two officers of the court is to maintain the delicate balance of the laws of our state. This is a clear violation of their oaths of office.
The first is the Honorable Judge Pam jackman Brown of the Supreme Court of the New York. The second is of the Honorable Gail Adams of the New York state court Family division. The later of the two accepted checks from a woman who had dementia, was cognitively impaired and unable to make decisions for herself. My mother, an elderly and financially abused woman, was physically unable to sign any document.
The other individuals involved in this documentary are Tonya Hobson Williams (Representing the Children of Ursula Brathwaite), Judy Mock court appointed representing Ursula Brathwaite, the law firm of Alter Barbaro hired by my cousin Charles Wilson. The last law firm accepted a check that was fraudulent (Knowingly accepting a check from a woman who was not only cognitively but physically limited and unable to sign a check or give consent)
Ms. Mock, the court appointed attorney for Ms. Ursula Brathwaite's neglected her duties as my mother's representative by not pursuing charges against Charles Wilson for financial abuse. This is despite sending our attorney Tonya Hobson Williams copies of over one hundred fraudulent checks, bank statements.
Menorah nursing home also neglected their duties as a skilled nursing facility by not reporting financial abuse as required by New York New State law. This is despite having copies of all documents that were forwarded to this writer by the attorney representing Ms. Brathwaite (court appointed). Charles Wilson was an employee of this facility at the time of the reported abuse (complaint number noted above).
The law firm of Alter and Balbaro showed neglect and compliance with abuse for accepting fraudulent checks and / or payments from a client that was cognitively impaired and physically unable to sign a signature. (Refer to medical records dated 01/05/2016). This is despite meeting this client post retainer agreement 05/21/2018.
In conclusion the above information is evidence of the concealment and noncompliance of the rules, regulations, and laws that cover elder care and financial abuse.
